ECA owns and operates three Senior Living Communities: Alma Via of San Rafael, AlmaVia of San Francisco, and AlmaVia of Camarillo; one Continuing Care Retirement Community: Mercy Retirement & Care Center; and the Villa at San Mateo, an Independent Senior Living Community.About This Career OpportunityThe Care Coordinator (Coordinator) provides care management services to members of the Congregation (Congregation) with the goal of assuring that appropriate health and psychosocial services are made available to and utilized by individual members.

The Coordinator is responsible for recommending, developing, implementing and supervising care programs for individuals identified by the Congregation which assure sound professional practices, provide safe and pleasant environments, respect the individual’s preferences while meeting their needs, and reflect the values of the Congregation.

The Coordinator will work with the designated Congregation liaison(s) to develop and implement a cost-efficient system of care, which addresses the immediate and future needs of Congregation members.

The Coordinator is responsible for establishing and maintaining effective communications with the Congregation, its members, members’ families, and other individuals and/or organizations providing care and services to the Congregation and its members.

Are You Qualified?Current licensure as Registered Nurse in the State of California or MSW.

Three years’ experience working in geriatric case management.

Demonstrated knowledge regarding aging issues, dementia care services, knowledge of community resources, and long-term care models.

Valid driver’s license and dependable means of transportation.

Ability to travel frequently in carrying out job responsibilities, with occasional travel throughout California and other locations as needed.Sensitivity and knowledge of working with older adults and the aging process and implications of aging in a social setting.

Excellent written and verbal communication skills.

Ability to identify need, develop and manage care and systems simultaneously.

Must understand health care environment and the psychosocial and medical context for older adults, with ability to communicate professionally, empathetically, and compassionately with elder population who may be ill, disabled, hospitalized, and/or experiencing loss.
